OAMCC	Aged 65 or above Man or woman Hospitalised within the last 90 days Managing at least five different medications (prescribed and over-the-counter medications explored during recruitment) Suffering from multiple chronic conditions4 Living alone or in a couple, in a rural or urban area With or without support from a Community Healthcare Centre	Not able to speak and understand French
Informal caregiver	Designated by the OAMCC as the most significant informal caregiver involved in medication management Aged 18 or above	Not able to speak and understand French
Professional caregiver	Designated by the OAMCC as having a key role in medication management	Student Apprentice
